Understanding the EHCP
We begin by reviewing the EHCP carefully, ensuring we have a clear and accurate starting point.

Keeping Information Safe
All information is handled securely and anonymised, so we can focus on support while protecting each learner’s identity.

Step 3:
Gathering Key
Information
We bring together information from the EHCP, assessments, and tools such as the SDQ to build an initial picture.

Step 4:
Identifying Strengths and Needs
We look at what’s going well, alongside areas where additional support is needed, creating a clear and balanced overview.

Step 5:
Understanding the Whole Child
Using a holistic approach, we explore patterns, experiences, and the wider context around each learner.

Step 6:
Planning the Next Steps (IEP)
We create a focused plan with clear, achievable goals tailored to the learner.

Step 7:
Putting Support in Place (ISP)
Support is personalised, with clear strategies to help each young person re-engage, stabilise, build and transition.

Step 8:
Reviewing and Adapting
We regularly review progress and adjust support as the learner grows and develops.
